We customize our care plans with activities that take place both inside and outside the home. Whether you need weekly, monthly, or annual services, we are here to meet your needs.
Errands, Shopping, Appointments
Errands, groceries, doctor's appointments, even social outings. YSC provides transportation to and from doctor appointments, therapy centers and other care facilities, but we will wait with clients, ensure discharge and care instructions are clear, and take them back safely home. This allows for less stress for client and the family and has proven to have better health outcome for the client.
Home & Wellness Service
Peace of mind calls! We can call daily, weekly for wake-up calls, "did you take your pills?" calls, wellness checks or just a brief conversation with a friendly voice. YSC care manager could also visit in home and maybe an expected visit from plumber or cable guy, or electrician or DME delivery so your loved one is not vulnerably home alone. We will insure safety as well as the job is done right.
Hospital or Rehab Discharge to Home
Often, a patient is quickly discharged from a hospital or rehab stay and the family can't take off of work, doesn't live close by and worried about loved one coming home safely. YSC will drive the newly discharged client home and help to safely transition them back at home or where ever they call home.. Assist in prescription fills, grocery shop, help in getting needed services started - like Home Health, therapy schedule, etc. ​
Concierge Service
Your Senior Concierge Service is available and absolutely client-focused. The client will need a care plan tailored to suit that very specific senior. Weekly visits to help with everyday tasks that might have become worrisome and difficult, trips to visit a friend, help them read and go through mail, etc. If you or your loved one needs anything - especially if not listed, just ask!
